OFDA Associate Membership Application

The Individual applicant listed on this application is applying for Associate Membership in the Ohio Funeral Directors Association. (“OFDA”).
Eligibility: The following individuals shall be eligible for Associate Membership in the Association:
1. Funeral directors duly licensed under the laws of the State of Ohio, who are in the profession of funeral directing and who are not owners of or are not employed by a funeral home;
2. Widows and widowers of deceased licensed funeral directors or embalmers or dual licensees or Associate Members who are members in good standing of the Association upon their death;
3. Trade embalmers licensed under the law of the State of Ohio;
4. Funeral directors licensed under the laws of the State of Ohio who are not presently engaged in the profession of funeral directing;
5. Funeral directors licensed under the laws of states other than Ohio, who are duly qualified members in good standing of their state associations; and
6. Retired funeral directors and/or embalmers who are not employed by a funeral home or other entity
An applicant must receive an affirmative vote from the OFDA Executive Director and Secretary/Treasurer in order to be admitted into membership.
Application: To complete the application, please fill in all of the information requested below. Following OFDA staff processing, the application will be reviewed by the OFDA Executive Director and Secretary/Treasurer.
Dues Schedule:
Individual dues: $110 annually
Retiree dues: $80 annually
Upon approval of your membership application you will receive an invoice to pay your dues based on a prorated amount depending on the time of year your application is approved.
Annually, OFDA members are mailed dues invoices in November. Deadline for remittance is February 28th.
Terms and Conditions: The applicant agrees as a condition of membership that the applicant will abide by the OFDA Bylaws that have or may be adopted by OFDA. Applicant understands that by providing their mailing address, e-mail address and telephone number, they consent to receive communications sent by or on behalf of OFDA.

1) Applicant is a funeral director who is duly licensed in Ohio, is actively engaged in the profession of funeral directing and is neither an owner nor an employee of a funeral home (Individual).
2) Applicant is a widow or widower of a deceased Affiliate or Associate Member who was in good standing as a member of OFDA at the time of his or her death (Individual).
3) Applicant is a trade embalmer licensed under Ohio law (Individual).
4) Applicant is licensed as a funeral director under Ohio law, is not retired, but is not actively engaged in the profession of funeral directing (Individual).
5) Applicant is licensed as a funeral director under the laws of one or more states other than Ohio and is a duly qualified member of the state funeral directors association in the state where the applicant is licensed (Individual).
6) Applicant is a retired funeral director and/or embalmer who is not employed by a funeral home or other entity (Retired).
